don't capitalise a generic determiner (a, the) or a conjunction (and, or) -- but i have never seen fixed rules for how to do it. you just get used to it, from book titles

there are about three *VERY VERY RARE* surnames that begin with small letter, like "ffoulkes", some medieval-historical french reason. i've never met anyone with those surnames.
